Membership & Licence Application

To apply for membership with Katsu-Mi Kai, please download and complete the application form below. The document contains both the Licence Application/Student Declaration and Membership Application Form.

Please complete all relevant sections and sign the declarations before returning your completed application using the upload form below.

On iPhone/iPad: Open the downloaded PDF, complete the fields and signature, then save a copy to Files before uploading it.

On Mac/PC: Download the PDF, complete and sign it, then save the completed copy before uploading.

Prefer to complete it by hand? You can print the application form, complete and sign it manually, then scan or photograph the completed pages and save them as a PDF before uploading.

Once you have completed and signed both pages, upload your saved PDF below. Please make sure you are uploading the completed version rather than the original blank form.

Alternatively, you can print the application form, complete and sign it manually, and hand the completed form directly to your Sensei at your next training session.
